Each of the authors of a manuscript is asked to fill in the following form attesting their authorship of the paper and confirming the disclosure of any declarable associations. Download the form, fill it in, sign (it can be signed electronically or by hand) and upload it to the COMET system or send by This email address is being protected from spambots.
